What is the difference between VAL and VAN codes?

VAN codes come from Vanguard, Riot's anti-cheat; VAL codes from the game and the Riot Client. The number is what matters: look it up in the table.

Does an error code mean I am banned?

Almost never. Only 61 (banned from playing) and 152 (hardware ban) are bans; the rest are connection, client or Windows settings problems.

Riot's servers or my PC?

Check Riot's status page first: if the servers have a problem, waiting is the fix. If they are fine, the steps in the table are for your PC.
